Pain in big toe- what is it?

I am not sure, but am gonna throw something out there. Gout is a disease that usually starts with pain in the big toe. Seriously. No injury or anything, but severe pain. It hurts to move, because there is inflammation and a buildup of uric acid crystals in the joint. Later it may progress to other joints, like the knee. It is easily treatable if diagnosed.
